You can go this way:
For categories 1 and 2
- Do a bulk edit and assign one carrier to all items inside categories.
- Create shipping box with flat rate type and assign that carrier with fixed price of $199 against it.
For categories 3 and 4
- Create a custom carrier (Rename one which is not used).
- Assign it to all items inside categories via bulk edit.
- Do a batch update for those items and assign flat rates for them (You can use Excel formula to make 10% of Web Price for all the items in the sheet).
For categories 1 and 2
